The group or entity must be registered with HCA and have a core supplier agreement with the Agency. Non-billing apps are intended for providers who provide services to Apple Health (Medicaid) customers as part of a managed healthcare organization or behavioral health organization (not under the Apple Health paid program) or providers who register for the sole purpose of becoming an order provider, expeller and prescriber. Hospitals and providers must, within five years of the previous revalidification, complete a revalidation for each National Practitioner Identification (NPI) number associated with their institution or group. Validation includes completing and uploading an updated basic vendor agreement, exclusion statement, IRS W9 form, and additional documents, depending on the nature of the facility or vendor. All billing providers must complete and sign a basic supplier agreement and other necessary documents with the Agency.
